• Polo Shirts ~ US$15.00 (Kindly indicate size for ordering)

Delegates are requested to return Registration Forms by the date given, as this is necessary to facilitate the ordering and printing of polo shirts and the timely preparation of the Conference Kits.

PAYMENT IN FULL IS REQUIRED ON OR BEFORE JULY 15TH 2008.

A late fee of US$10.00 will be applied after the July 15th deadline.

Payment by cheque will not be accepted after July 15th 2008 deadline.
